Community Programs Specialist II/Chesapeake Thrives Coordinator

The purpose of this position is to plan, organize, and provide coordination to the Chesapeake Thrives Plan for Health and Well-Being and its related programs, partners, and funding. This Plan provides information, referrals and community impact systems that strengthen families, promote resident safety and well-being, This position provides supervision for the Thrives’ Case Managers and other staff as needed. In addition this position:
- Creates and maintains strategic partnerships within the city and the community to ensure the eight priority areas’ strategies and initiatives are current, research based, and data driven
- Convenes and facilitates workgroups addressing the priority areas of the Community Well-Being Plan: Workforce Development, Poverty and Economic Supports, School Age Program Support, Early Childhood, Aging in the Community, Health, Behavioral Health, and Housing.
- Implements goals of the Thrives Plan through programming, initiatives, advocacy and community education.
- Ensures eight priority areas are meeting benchmarks and are current or updated as needed.
- Collaborates with community partners to set, track, implement and reach desired outcomes.
- Provides presentations on the plan and its progress in the community as needed.
- Oversees No Wrong Door Chesapeake Thrives Prevention case management team.
