SHERIFF JAIL

Will work collaboratively with the Sheriff’s Office and the Sheriff’s Office’s contracted medical provider to screen and assess the psychiatric, medical, social services, employment, and residential needs upon entry into the Chesapeake Correctional Center, (CCC.)

Will be responsible for coordinating pre-release and post-release case management for CCC inmates with mental health, critical care medical, and substance use disorder needs to ensure a smooth transition to the community and services.

Will collaborate and coordinate with Chesapeake Integrated Behavioral Health’s community-based services and community partners to refer and link inmates with resources to support community re-entry, to include transitional housing or other housing support, and minimize recidivism.

Creates the development of a written discharge plan with prioritized goals and objectives that reflect assessed needs of the inmate. Works with the inmate on the discharge plan while incarcerated at the CCC and for at least 30 days in the community. The discharge plan examples are assisting inmates with a SMI resident with obtaining photo identification, birth certificates, apply for Medicaid and/or GAP applications/reinstatement, apply for Social Security disability/SSI assistance, assist with emergency food and clothing as approved through the Inmate Jail Program Fund, etc.
