The Mobile Integrated Healthcare (MIH) Program is an innovative and evolving model of community-based, patient- centered healthcare designed to provide effective and efficient services to Chesapeake’s residents. Under general supervision, the MIH Family Services Specialist II facilitates, coordinates, and provides case management supervision for multiple social work service programs administered by the City of Chesapeake Human Services Department. May coordinate specific services provided by the department with overall responsibility for the daily activities and outcomes.

Typical Tasks:
  • Supervises Social Workers and support staff, including training, assigning and evaluating work.
  • Assists agency clients with personal, social, health and economic needs and addresses barriers to healthcare including transportation, insurance coverage, financial status, and others.
  • Conducts interviews or investigations and makes client/resident and family assessments; formulates and implements plan of action; documents progress.
  • Interacts/coordinates with other agencies, organizations, or the courts to provide services for clients/residents.
  • Monitors program compliance with applicable guidelines, regulations, and laws; monitors referrals and notifications.
  • Coordinates services with other agencies such as hospitals, behavioral health and assisted living facilities, and other community services.
  • Provides information to public on eligibility requirements and available services for assigned area.
  • Supplies information about agency and/or community resources and facilities to neighborhood residents.
  • Able to work independently in and out of the office in the neighborhood setting while maintaining regular communication with supervisor with regards to preparing and developing and executing service plans.
  • Coordinates emergency services/crisis intervention for clients.
  • Assesses the need and extent of protective services required and-in collaboration with the client, their support systems, and other public and private service providers-arranges for the necessary services to prevent/ameliorate the abuse, neglect or exploitation.
  • Assesses the need for guardianship/conservatorship and facilitates the process to include medical documentation, preparation of information required for petitions, testimony in court and advocacy.
  • Assesses the need for mental health intervention.
  • Performs routine office tasks such as dictating case notes, data entry, typing correspondence or forms, filing, faxing, telephoning, or photocopying.
  • Conducts training classes for service providers; attends professional workshops and seminars to improve professional skills.
