Human Services Assistant II - AS/APS Intake (Shenandoah Valley)

This is a part time professional/para-professional position under the supervision of a Family Services Supervisor that performs responsible professional work in the support and assistance of those involved with the Adult Services/Adult Protective Services unit. (AS/APS) Responds to calls to the AS/APS Intake line, obtaining information both in-person and over the phone, meets with AS/APS walk-ins, links CPS walk-ins with CPS intake, enters mandated information and assignment of AS/APS cases into the PeerPlace case management system, obtains financial information to determine eligibility for AS services, maintains intake log, assesses for companion waitlist and reassesses those individuals every six months, and maintains the payee waiting list. Initiates follow-up calls to guardians who are overdue with submission of annual/initial/or final guardian report to the agency. Assists Family Service workers with case responsibilities, providing emergency assistance to family service workers on an as-needed basis, providing referrals for community based services, maintaining communication with clients and professionals, assisting in the completion of monthly visits for ongoing APS clients, maintaining case records, and performing other assigned duties. Other duties as assigned.
