Performs responsible, equitable, and professional casework in a school setting and within an inclusive team environment in accordance with established social work/counseling principles. Uses an asset-based approach to engage with the client system to support and assistance of Bright Stars families with personal, social, health, educational, and economic needs. Empowers the family to the highest level of self-sufficiency; requires minimal supervision. Articulates, supports, and applies the high-performance principles (continual quality and service improvements, participatory involvement, teamwork, data collection, and analysis) of the department and County.

ESSENTIAL FUNCTIONS:
  • Plans and coordinates the recruitment, application, and selection processes for students for the Bright Stars Program.
  • Plans and implements a process for parent and family involvement in the Bright Stars Program inclusive of family night, field trips, special events, etc.
  • Implements the dental and health component of the Bright Stars program on the individual school level.
  • Collaborates with elementary grade teachers to provide early childhood intervention and family support for Bright Stars preschool alumni.
  • Actively participates in the school case management team.
  • Ensures follow-up with Bright Stars students and families.
  • Follows the Bright Stars alumni through grade five to provide evaluative data and participate in related research efforts.
  • Serves as liaison between Bright Stars Program Coordinator and school site staff as needed.
  • Works collaboratively within multiple systems inclusive of schools, DSS, families, and community.
  • Recruits, engages, and provides services to families through school and other referrals.
  • Develops comprehensive assessments of the strengths and needs of Bright Stars children and their families.
  • Works with the family to develop a responsible service plan based on the assessment.
  • Assists and supports families in their use of agency, school, and other resources.
  • Provides direct support and counseling by teaching family problem-solving strategies and providing tools for families to address areas of need.
  • Maintains professional confidential case records according to state and federal law and policy.
  • Provides accurate and timely updated entries into the required information systems.
  • Advocates for families in the educational process.
  • Participates in the identification of social problems and advocates for the development of needed resources.
  • Interprets agency programs and policies to other agencies and the community.
  • Attends and participates in related departmental and school meetings and training.
  • Serves when called upon to assist in emergency situations, including emergency sheltering.
  • Performs related work as required.
